Whether Obamacare is upheld, struck down, or something in between, the high court's decision will shape the future of American health care.
Reform advocates worry about re-fighting the battle with the nation politically polarized and congress hopelessly gridlocked.
Opponents argued the law crossed the constitutional line when it required all Americans to buy health insurance or pay a penalty.
"If we win on the individual mandate, Obamacare in totality will fall," said Greg Abbott, Texas Attorney General.
There are political implications in this election year.
Mitt Romney benefits either way, because he's vowed to dismantle the health care law.
But for President Obama, the health care law is his signature achievement and, perhaps, his legacy.
